The front page of a recent letter from the Department for Work and Pensions (DWP), and the page that states the type of benefit received

A letter sent from the DWP showing current claim dates, plus the type of benefit being claimed

A memo from the DWP stating name, address, the benefit claimed and that it is current

The files must be smaller than 10MB in size each. This form will allow a maximum of two files to be uploaded.

If you are unable to upload documents, you can post a photocopy to: Travel & Transport, Norfolk County Council, County Hall, Norwich NR1 2DH. Photocopies must be labelled with your name, reference number (which you will receive at the end of this application process), and Sixth form /college attending.

The information collected on this application may be passed to learning establishments and transport providers to assist with the planning and operation of journeys and to verify details. Data will be held for up to 6 years after the student has reached the statutory school leaving age (16) to comply with contract and audit requirements, after which time it will be securely destroyed.
