The files must be smaller than 10MB in size each.  This form will allow a maximum of two files to be uploaded.

If you are unable to upload documents, you can post a photocopy to: Travel & Transport, Norfolk County Council, County Hall, Norwich NR1 2DH. Photocopies must be labelled with your name, reference number (which you will receive at the end of this application process), and Sixth form /college attending.

Additional information (optional)

The information collected on this application may be passed to learning establishments and transport providers to assist with the planning and operation of journeys and to verify details. Data will be held for up to 6 years after the student has reached the statutory school leaving age (16) to comply with contract and audit requirements, after which time it will be securely destroyed.

By submitting this form you confirm that transport is required for the above mentioned student to the above sixth form/college for September.
